Requirements to Receive CPUI:
Start your journey to becoming a CPUI today! Please print and complete the application provided, then submit it to PICC Excellence for pre-approval. Simply upload the application to your online account or send it via USPS. Once you have confirmed that you meet all the qualifications, you are ready to purchase your CPUI certification.
Note: Application for CPUI PICC Certification - requests personal information (name, address, etc.) and requires a copy of a professional license.
Verification of PICC Experience requires a record of PICC training history, PICC experience and proof of 8 CEs of basic PICC education. This form requires a notarized signature by an employer or supervisor attesting to the validity of the information. Then the candidate must sign to swear that the information is true and the signature must be notarized to validate identity.
Supervised Insertion Competency Checklist requires the candidate to perform a supervised insertion allowing the preceptor to assess specific step-by-step procedures to ensure the candidate is following current guidelines as established by INS and CDC. Both the preceptor and candidate must sign the form upon completion of the assessment. Once your application is approved and purchased you will have 60 days to study and take the Remote Proctored CPUI examination, from home or work. PICC Certification Study Manual - This is the manual the candidate can use as a study guide prior to taking the 200-question certification exam.
